    HA-FEM: Labia Majora Filler

The volume loss nobody talks about. HA-FEM (Hyaluronic Acid Feminine Intimate Treatment) restores lost fullness to the outer labia with a long-lasting hyaluronic hydrogel — privately, gently, and always with a female chaperone present.

Labia majora filler (HA-FEM) is a doctor-led treatment that restores lost volume to the outer lips of the vulva using a long-lasting hyaluronic acid hydrogel, placed by blunt cannula under local anaesthetic. At Cosmedocs Harley Street it costs £950–£1,450 by grade, with a female chaperone always present.

    The unspoken concern

It is rarely about vanity. It is about how a woman feels in her own body.

Most women will happily tell a friend they are thinking about under-eye filler. Very few will say they have noticed their outer labia looking flatter, looser or more wrinkled than they used to. Yet it is one of the most common changes we are asked about in consultation — usually quietly, at the end of an appointment booked for something else.

Women describe a quiet sense of grief at losing fullness; self-consciousness in intimate moments; feeling older than they are; anxiety about swimwear and fitted clothing; and, because it is never discussed, a belief that they are the only one. They are not.

The outer labia act as a cushion. When that cushion thins, friction from cycling, running or tight clothing can cause soreness and dryness — and the discomfort keeps the area front of mind. The clinical literature links outer labial volume loss to dryness, irritation and discomfort during intercourse, with real effects on confidence and intimacy.

The good news: this is a volume problem, and volume is what we do every day. The same principles we use to restore a hollow cheek or temple apply here — with extra anatomical care, privacy and a female chaperone in the room for every treatment.

    Anatomy

Understanding the labia majora: skin, muscle and fat

The labia majora are the outer lips of the vulva. Their job is protective: they cover and cushion the inner lips and the vaginal opening from friction, pressure and irritation.

They are built in layers, much like the cheek: pigmented outer skin over a dense, vascular dermis; a thin sheet of smooth muscle (the labial dartos); then a superficial fat layer and a deeper fibro-fatty cushion — the labial fat pad — which gives the outer lips their rounded shape.

When we talk about volume loss, we mean the fat layers shrinking. The skin above has less to rest on, so it creases and folds — exactly as a cheek does when its fat pads deflate. That is why a surface cream or laser alone cannot fix it: the missing ingredient is volume, and it sits underneath.

    When it starts

First signs usually appear in the late twenties — years before most women expect. By the mid-thirties to mid-forties it is often obvious, and after menopause it can progress quickly.

    What speeds it up

Natural ageing, falling oestrogen, pregnancy and breastfeeding, sudden weight loss (including weight-loss injections), childbirth and daily friction.

    Honest assessment

The Cosmedocs grading system for labia majora volume loss

We grade outer labial volume loss from 1 to 4, and we treat Grades 1 and 2 with filler. Grading gives you a clear, honest answer about where you are and what filler can realistically do. Ages are a guide, not a rule — your grade is set at an in-person examination, never from a photo.

    Grade 1Treated at Cosmedocs

Early volume loss · Late 20s to 35

Slight flattening of the outer lips and fine crinkling of the skin. The ideal time to treat: a small, precise volume restores the contour completely.

    Grade 2Treated at Cosmedocs

    Moderate volume loss · 35 to 45

Visible deflation; the skin looks softer and looser, especially standing. Restoring the fat compartment lifts and smooths the skin above it.

    Grade 3Specialist assessment

    Marked volume loss · 45+ / menopause

Clear hollowing, folds and laxity. Filler alone may not give the result you want, so we assess you individually and discuss the options.

    Grade 4Specialist assessment

    Severe atrophy · Post-menopause

Very little fat remains and the skin is lax. This usually needs a different approach, such as fat transfer or a surgical opinion, and we will refer you if that is right for you.

    How HA-FEM works

    Labia puffing at Cosmedocs

Labia majora filler — often called labia puffing or a labial puff — replaces lost volume in the outer lips with a long-lasting, natural hyaluronic acid hydrogel. It is non-surgical, done under local anaesthetic, and you walk out the same day.

Hyaluronic acid is a sugar your body already makes; it holds water and gives tissue its plumpness. The hydrogels we use are cross-linked for durability and chosen specifically for the softness and movement of the intimate area. They restore volume and hydration together, integrate smoothly with the tissue, and — importantly — can be dissolved with hyaluronidase if ever needed. That reversibility is one reason we prefer them to permanent or semi-permanent products.

The skill is in the placement. We work with a fine blunt cannula through a small number of entry points, laying the gel in thin threads within the fat compartments rather than as a single deposit. Just as in the face, building volume layer by layer gives a soft, natural contour that moves with the body and does not look “done”.

    Step by step

    Your appointment

    1. Private consultation
We talk through what you have noticed, your medical history and your goals, and explain the options — including not treating.
    2. A doctor examines you, with a female chaperone present, and confirms your grade.
    3. We ask you not to shave or wax the area for a few days beforehand. On the day the skin is thoroughly cleansed.
    4. A topical anaesthetic cream, then a tiny local anaesthetic injection with a very fine microneedle, then the filler itself — which contains lidocaine, so the area keeps numbing as we treat.
    5. Hydrogel is placed by cannula in fine layers, then gently moulded and checked for symmetry. Most treatments take under an hour.
    6. We see you again to check the result and healing.

    Safety, privacy and dignity

A female chaperone is always present. No exceptions.

A female chaperone is present for every intimate consultation, examination and treatment at Cosmedocs. This is mandatory, not optional. You will never be alone with a practitioner during any part of the process, and you can ask to pause or stop at any point.

Every treatment is carried out by an experienced medical doctor with detailed training in the anatomy of this area, in a private treatment room.

Expect some swelling, tenderness and possibly bruising for a few days; the final shape shows once swelling settles. For the period your doctor advises, avoid intercourse, tight clothing, cycling, the gym, swimming and hot baths. Serious complications such as infection, lumps or a blocked blood vessel are rare, and are one reason this treatment should only be done by an experienced medical practitioner.

    Who is suitable

Women aged 18 or over in good general health, bothered by outer labial volume loss, dryness from friction, or loss of contour at Grade 1 or 2.

    When we will not treat

    • Pregnancy or breastfeeding
    • An active infection in the area, including thrush, herpes or genital warts
    • Inflammatory skin conditions near the treatment site
    • Known allergy to any ingredient in the filler
    • Permanent or semi-permanent filler already in the area
    • Recent surgery in the area, or active cancer nearby
    • Certain autoimmune conditions, which we will discuss at consultation

We will also be honest if we think filler will not give you the result you are hoping for, or if your concern would be better served by a GP, gynaecologist or other support first.

    Transparent pricing

    HA-FEM cost at Cosmedocs

Pricing is set by your grade, because your grade decides how much hydrogel and time the treatment needs. Your price includes consultation, examination with a female chaperone, the treatment itself and your review appointment. Your grade — and therefore your price — is confirmed at your in-person consultation.

Treated early, the gap between how things are and how they were is small — and small gaps are easier, cheaper and more natural to close. Maintained this way, the change is never dramatic, the amount of product stays modest, and the result always looks like you.

    Labia filler FAQs

These answers stay present in the page for patients and search engines, even when a panel is closed.

Not quite. Many people search for vaginal filler or intimate filler, but the vagina is the internal canal. Labia majora filler treats the outer lips of the vulva, on the outside of the body.
Labia puffing, or a labial puff, is the popular name for labia majora filler: restoring lost volume to the outer lips so they look fuller and smoother. Some people search for it as “puffy labia”.
No. A labioplasty is surgery that reduces the inner lips (labia minora). Labia majora filler adds volume to the outer lips and involves no cutting. Restoring outer volume can make prominent inner lips look less noticeable, but it does not reduce them.
In our experience the first changes often appear in the late twenties, with more visible loss between 35 and 45 and faster change around menopause. Weight loss and hormones can bring it on earlier.
We numb in three steps: a topical cream, then a single tiny anaesthetic injection with a microneedle, then filler that contains lidocaine. Most women describe pressure rather than pain. The area may feel tender for a few days afterwards.
That is the aim of every treatment. Placing hydrogel in fine layers within the fat restores your own contour rather than creating a new shape.
Yes. A female chaperone is present throughout every intimate consultation and treatment at Cosmedocs, without exception.
Yes. Hyaluronic acid can be dissolved with an enzyme called hyaluronidase if needed.
