Jowls are the soft sagging that appears just in front of and below the jawline, where the clean edge of the jaw stops being a clean edge. They are caused by three things happening together: fat pads descending, ligaments and skin losing support, and bone resorption at the jaw and chin reducing the shelf that everything used to sit on. Fat is only one part of it, which is why fat-only treatments so often disappoint.

There is no single best treatment — it depends on whether your jowl is mainly volume loss, mainly laxity, or both. Structural jawline and pre-jowl filler restores the bony shelf and blunts the shadow. Cog (barbed) PDO threads physically reposition descended tissue. Yes, with the right thread. Mono threads are fine, smooth threads used in a mesh to improve skin quality and give gentle tightening — they do not lift. Cog threads carry small barbs that grip descended tissue and reposition it upwards, anchoring in the temporal or pre-auricular plane. Cogs are the lifting thread. Results last roughly 12 to 18 months and are repeatable.

Surface devices heat the skin from the outside and are limited by what the skin surface can tolerate — the first couple of millimetres. That improves skin quality but does not lift. Endolaser passes an optical fibre thinner than a hair beneath the skin and delivers dual-wavelength energy (1470nm and 980nm) directly into the deep dermis, fat and fibrous septae. Endolaser reduces a measured volume of fat and tightens tissue in the same pass, is doctor-delivered under local anaesthesia, and once a full course of injections is costed the overall price is broadly similar. We would rather offer the treatment that supersedes it.

Jawline filler lasts roughly 15 to 18 months in this area because there is little muscle movement. Cog threads last around 12 to 18 months.
